Assistance programs may require some of the information listed below, so try to have it available before beginning the application process.
Information you give us will be used to help TDHS determine benefit levels. If you do not have this information, or if you choose not to provide it, your application could be denied.
- Job employment details and/or pay stubs
- Your bills
- People in your home
- Checking and savings account information
- Relationship verifications (e.g., marriage certificate)
We will need the most recent versions of these verifications. If you are having trouble getting the verifications, don’t understand what is needed, or need help getting the verifications, contact TDHS.
